Patient Navigation Services
During all stages of your care — from the moment you receive a cancer diagnosis through treatment and beyond — you have needs, and they’re important. Our patient navigators deliver solutions that address all aspects of your health, including physical, emotional and spiritual.
We offer:
- A psychosocial stress screening immediately following cancer diagnosis to examine your psychological well-being and determine if you might benefit from psychosocial care
- Coordination of care before and during treatment through surviving cancer or hospice
- Education to decrease any anxiety about the disease and diagnostic tests and treatments you may undergo
- Referrals to supportive care services that address your physical, emotional and spiritual health, such as nutrition counseling, gentle yoga and chaplain services
