Trinity Health’s pre-admission program is designed to streamline your admission and help ensure a smooth and worry-free hospital stay.
Pre-admission preparation may include blood tests, x-rays, medical history, physical exam, and patient education. These should be completed within a window of no more than two weeks and no less than 48 hours in advance of any surgery to avoid cancellation or delays in your surgery date.
Preparing for Your Hospital Stay
Please bring all of your insurance information with you, including names of companies, policy numbers and co-pays.
Bring a list of prescription and non-prescription medications that you are taking. This includes vitamins, over the counter medications, and herbal supplements.
- These products may interact with anesthesia or affect how well your blood clots. Your healthcare team can make adjustments if they know what you are taking.
Consider completing an Advance Healthcare Directive document (see the section on making Healthcare Directives). If you already have one, please bring a copy with you.
For your Privacy
Upon admission you will be assigned a personal identification number or PIN, which you may share with whomever you choose. To protect your privacy, no information about your condition will be released over the telephone – even to family members – unless the caller provides the correct PIN number.
Leave all valuables at home
We encourage you to leave your valuables at home or send them home with your family. For your convenience, safekeeping of valuables is available. Trinity Hospitals is not responsible for the loss of or damage to any personal property. This includes but is not limited to clothing, jewelry, dentures, hearing aids, glasses, cash, and cell phones. We recommend that the patient keep no more than $5.00 with them.
