Trinity Health North Dakota uses Cerner Health to provide a secure username and password used to access your patient record information. Self-Enrollment for My Patient Portal
Self-enrollment is available for Trinity Health, Minot, North Dakota patients who are age 14 or older who don’t currently have access to My Patient Portal, which is an online portal where you can view your medical information and connect with your health care team. How to obtain your login for the Patient Portal
When you appear at a clinic or have a hospital visit, you will be asked if you would like to sign up for the patient portal. Provided you give us a photo ID and a valid email address, you will receive an email invitation with a link that connects you to another secure site where you set up your portal user account. Once you’ve followed the link you will be given directions on how to complete the registration process.
If you are a Trinity patient but haven’t visited your doctor recently, you may stop by one of our clinic locations with photo identification and request an invitation to sign up for the portal. This face-to-face visit is necessary to prevent medical identification theft.
What if I forget to follow the email link?
You must follow the link to complete the registration process. If you do not complete the registration process within 90 days, the link allowing you to register will expire. You will then need to appear in person again to restart the process.

The Patient Portal is password protected, and all messages are encrypted to ensure security. No one will be able to view your health information except you or someone you have authorized, and your care team.
