Diabetes Prevention Program – Weigh 2 Change Session Zero
August 27 @ 12:00 pm - 1:00 pm
Weigh 2 Change Session Zero, an informational session, will take place on August 27 from 12-1 p.m., Trinity Health Medical Arts, Skywalk A conference room.
Diabetes can be prevented but not cured. Take control of your health and start making positive lifestyle changes with the help of a trained lifestyle coach and fun, engaging support groups. You will learn about the yearlong program and its goals, complete a “Readiness to Change Questionnaire,” and you can ask questions to see if this program is right for you.
Space is limited, RSVP to 701-857-5268. If you would like to participate in the program, but cannot attend the informational Session Zero, call 701-857-2850 to do an over-the-phone individual session.
Cost: People who decide to commit to the program and are not covered by Medicare will be charged a fee of $220, which covers education sessions and materials for the entire year. Classes will start September 2024.
Eligibility for Participation:
- At risk for or have prediabetes
- At least 18 years of age
- Overweight
- Do not currently have diabetes
- Not pregnant
