Home Health provides a wide range of healthcare services in the home to patients recovering from illness or injury, or in need of continuing care.
What is Home Health?
Home health care describes treatments provided by a variety of skilled healthcare professionals in the patient’s home. That could mean physical and speech therapy following a stroke, or diabetic wound care. The overall goal is to restore health and function after an illness or injury.
This kind of medical care costs less than inpatient rehab or a nursing home and is proven to reduce hospital readmission.
To help you regain strength and independence, Trinity assembles a team of nurses, medical social workers, therapists, aides, and other skilled professionals. They work with you and your primary care provider to create a plan that addresses treatment goals. Together, you evaluate what works and if the plan needs to change.
