Home Health Services

Examples of skilled home health services offered by Trinity include:

• Wound care (including dressing changes)
• Catheter and ostomy support
• Therapy to regain mobility
• Intravenous or nutrition therapy
• Check/monitor vitals (blood pressure, pulse, etc.)
• Blood draw
• Post-stroke swallowing therapy
• Patient and caregiver education
• Accessing community resources

Qualifications for Home Health

• Documented need for skilled nursing services and/or therapy
• Physician’s order stating the need for nursing or therapy services
• Patient doesn’t qualify for inpatient rehabilitation or nursing home
• Unable to leave home frequently

Referral and Assessment for Home Health

Following a new diagnosis, hospital or inpatient rehabilitation stay, your primary care provider may recommend home health services. This is what happens between a referral and the first home care visit:

• Physician places an order with Trinity Home Health
• Intake specialist calls the patient to gather insurance information
• Home health nurse or therapist visits the patient at home to do an assessment and make sure they qualify for services
• If patient qualifies, a team member contacts them to discuss what services will be covered by insurance
• Following physician orders, a care plan will be designed and activated
• Start receiving in-home visits from your team
