Pre-Surgery Registration

Trinity Healths' preadmission program is designed to streamline your surgical admission and help ensure a smooth and worry-free hospital stay. Pre-surgical preparation may include blood tests, x-rays, medical history, physical exam and patient education. These should be completed within a window of no more than two weeks and no less than 48 hours in advance of your surgery to avoid cancellation or delays in your surgery date.

Preparing for Your Hospital Stay

Please bring all of your insurance information with you, including names of companies, policy numbers and co-pays.
Consider completing an Advance Directive document (see the section on Making Healthcare Decisions). If you already have one, please bring a photocopy with you.
Bring a list of prescription and non-prescription medications that you are taking. This includes vitamins, over the counter medications and herbal supplements. These products may interact with anesthesia or affect how well your blood clots. Your health care team can make adjustments if they know what you are taking.

For your Privacy

Upon admission you will be assigned a personal identification number or PIN, which you may share with whomever you choose. To protect your privacy, no information about your condition will be released over the telephone - even to family members - unless the caller provides the correct PIN number.

Leave all valuables at home

We encourage you to leave your valuables at home or send them home with your family. For your convenience, safekeeping of valuables is available. Trinity Hospitals is not responsible for the loss of or damage to any personal property. This includes but is not limited to clothing, jewelry, dentures, hearing aids, glasses, cash and cell phones. We recommend that the patient keep no more than $5.00 with them.

Arrange for a ride home

You must make definite arrangements for a responsible adult (over the age of 18) to drive you home and stay with you overnight. You cannot be discharged alone in a taxi. Driving a car, operating any machinery or power tools is PROHIBITED for twenty-four (24) hours after sedation or general anesthesia. Your surgery may be cancelled if you do not have an adult driver to drive you home. You must call if you cannot make these arrangements.
