Trinity CancerCare Center: Survivorship

The Survivorship program provides needs assessment (long and short term), educational information, resources, a variety of complimentary alternative medicine choices and health promotion for overall health and risk reduction.

In an effort to ensure that cancer survivors physical, social, emotional, functional, body image, and stress needs are met, the Survivorship Program enhances your oncologists care. Our goal is to address the long-term needs of cancer survivors and bridge the gap from when your treatment begins and ends.

You will be presented with a summary care plan at the end of your treatment. The care plan will include all of the information to include surgery, chemotherapy medication, medical providers, and any major side effects that occurred. This information will be useful to you and your primary care provider.

The Nurse Practitioner is currently assessing women with curable breast cancer and plans to expand the program every year. Certainly if this is something that interests you I would welcome any questions or provide any information that could be of benefit to you.
